For students applying for preparatory, undergraduate, or long-cycle programs in Poland:
1) A photocopy of the front page of the passport
2) Passport-sized photograph
3) Original diploma + apostille document attached to the original diploma

Points to consider in this document:
The apostille must be made on the name and surname of the principal whose signature is present on the diploma, or if the diploma has the names and seals of the branch manager/authorized person working at the Provincial or District Directorate of National Education, it must be done based on this person's area of authority.
The names of the authorized person or persons on the diploma document (front or back page) must match the names on the apostille document in item 2.
The apostille process is carried out through the District Office or Governor's Office.
When submitting the original diploma and apostille for school applications, the back side of the diploma and/or additional documents must always be included in the file.
The names of the principals on the diploma and transcript must match. If they do not match, the student must request a document from the school explaining the reason for this situation. This document must also be signed with a wet signature and stamped by the current school principal. (An example is available on the following pages.)
4) Certified English translation of the diploma document

Points to consider in this document:
- The names of the authorized individuals on the diploma document (front or back page) must match with the name and seal of the person who has international signature authority on the apostille document in item 2.
- The seal of the authorized individuals who signed and stamped the diploma document (principal/deputy principal/branch manager, etc.) must be translated by a sworn translator.
- At the end of the translated document, it should be included which language the translation was done from, who the sworn translator was, and, if possible, the translator's identification number. This statement must be in English.
- The translated document must bear the seal of the sworn translator.
5) Original transcript + apostille document attached to the original transcript
We recommend obtaining the transcript document directly in English from the school. In this case, no translation process will be needed.
However, some schools may not issue an English transcript. In this case, the student must obtain a Turkish transcript document. The document must be signed with a wet signature, stamped, and must be signed by the school principal.

Points to consider in this document:
- The apostille must be made on the name and surname of the principal whose signature is present on the transcript, or if the document contains the names and seals of the branch manager/authorized person working at the Provincial or District Directorate of National Education, it must be done based on this person's area of authority.
- The names of the authorized person or persons on the transcript document must match the names on the apostille document in item 2.
- The apostille process is carried out through the District Office or Governor's Office.
- When submitting the original transcript and apostille for school applications, the back side of the transcript and/or additional documents must always be included in the file.
- The names of the principals on the diploma and transcript must match. If they do not match, the student must request a document from the school explaining the reason for this situation. This document must also be signed with a wet signature and stamped by the current school principal. (An example is available on the following pages.)
If the student has received their transcript in Turkish from their school, the first step to be taken at this point is the apostille process. Then, the sworn English translation of the transcript document must be done.

6) Certified English translation of the transcript document

Points to consider in this document:
- The names of the authorized individuals present on the transcript document (front or back page) must match the name and seal of the person who has international signature authority on the apostille document in item 2.
- The seal of the authorized individuals who signed and stamped the transcript document (principal/deputy principal/branch manager, etc.) must be translated by a sworn translator.
- At the end of the translated document, it should be included which language the translation was done from, who the sworn translator was, and, if possible, the translator's identification number. This statement must be in English.
- The translated document must bear the seal of the sworn translator.
7) Eligibility Statement document

Points to consider in this document:
- The Eligibility Statement document can be filled out by the school on a computer or optionally printed and filled out by hand.
- The document must be signed with a wet signature and stamped by the school principal.
If the document cannot be filled out in English or cannot be signed by the school in a foreign language, it can be prepared and signed in Turkish. However, thereafter, the sworn English translation of the document must be obtained.
------------------------------------------------------------------------------
If the following situations apply;
• There has been a change of principal at the school where the student preparing the documents is studying or has graduated,
• If the newly appointed principal does not yet have signature authority due to the change of principal,
• The school has closed and been transferred to another school and education is continuing there,
The following documents must be prepared as additional documents by the school, signed with a wet signature and stamped, and submitted to the school during the application process.

There is no need for any apostille process for the documents. We recommend obtaining these types of documents in original English. If the documents are obtained in Turkish, sworn English translations must be made.
8) Language Certificate
There must be a language certificate that is valid internationally, in accordance with the list published by the Polish Ministry of Education as of July 1, 2025.
